Insurance and Billing

Co-pays are due at the time of your office appointment (unless it is your first visit after surgery).

Please bring your insurance cards with you as we need to make copies for your file.

We submit all claims for you to your insurance company, but it is your responsibility to verify insurance coverage prior to your first visit.

Coverage eligibility can be easily verified by calling the information number on the back of your insurance card.
