Referrals To Other Physicians

Because we are a specialty practice, all referrals must come from your primary care doctor. If one of our surgeons recommends that you see another physician to follow-up on a particular aspect of your health care, you must verify through your insurance company that the recommended doctor participates in your plan. If a completed referral form is needed, you must obtain it from your primary doctor before you go to that appointment. Our physicians will correspond with your primary doctor to advise him or her about the plan for your treatment.
